Abstract
Educators from St Luke’s Hospice (Harrow and Brent) helped care home practitioners to develop their confidence and knowledge around having advance care planning conversations with residents. An advance care planning workshop was delivered in 11 care homes across Harrow, followed by a reflective session that was held two weeks later. The reflective session highlighted a range of issues that practitioners identified as influential in terms of having conversations about advance care planning with residents and families. The cultural backgrounds of the health professionals and residents was discussed as an area that needs further consideration when supporting advance care planning education in care homes.
